This questionnaire asks about things that may have happened to you in your school, in your neighbourhood, or in your family. It will ask questions about some situations where you might have been hurt or afraid you were going to get hurt. All your answers will be kept strictly confidential. All your answers are private. We will not tell your parents, teachers, or anyone else about anything you have answered on this form.
